Waiver of Liability and Photo Release for Jiu Jitsu Gypsies
I hereby understand and acknowledge that the training and events held by Jiu Jitsu Gypsies may expose me to many inherent risks, including accidents, injury, illness, or even death. I assume all risk of injuries associated with participation including but not limited to falls, contact with other participants, effects of weather and environment, including high heat or humidity, and all other such risks being known and appreciated by me.
After having read this waiver and knowing these facts, and in consideration of acceptance of my participation in these events, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS WAIVE AND RELEASE Jiu Jitsu Gypsies, its officers, employees, organizers, representatives and successors, as well as the host school from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in Jiu Jitsu Gypsies events.
I do hereby give Jiu Jitsu Gypsies, its assigns, licensees, and legal representatives, the irrevocable right to use my name, video, picture, portrait, likeness, or photograph in all forms and media in all manners, including composites, for advertising, publication, or any other lawful purpose, and I waive any right to inspect or approve the finished product, including written copy, which may be created in connection therewith.
By completing this form, I indicate that I have read and understand this Waiver of Liability and Photo Release. I am aware that this is a waiver and a release of liability, and I voluntarily agree to its terms. It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be deemed as a release, waive, discharge, and convention to sue the above named releasees.
COVID Release of Liability
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be at risk of being exposed to or infected by COVID-19 by attending activities with Jiu Jitsu Gypsies (JJG) that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 while at a JJG event may result from the actions, omissions, or negligence of myself and others, including, but not limited to, JJG employees, classified staff, coaches, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or my children (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Jiu Jitsu Gypsies, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or elating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of JJG, its employees, agents, and representatives, whether a COVID-19 infection
occurs before, during, or after participation in any JGG activity.
